Home Health Care

Who is home health care for?

  • someone who lives at their own home and requires regular nursing care, therapies, or personal care 
  • someone who has difficulty leaving their home to receive care


What is home health care?

  • when health care personnel make visits to your home to perform skilled health-related tasks, such as:

                 - wound care

                 - tube feeding

                 - medication management

                 - physical/occupational therapy

                 - help with recovery after procedures or surgeries

  • ordered by a physician as part of a care plan
  • costs usually covered, at least in part, by insurance


When should home health care be considered for care?

  • when someone is still capable of living in their own home, but has specialized needs that can be managed by visits from healthcare personnel
  • after discharge from the hospital or other health care facility, but skilled services are still needed to continue care


*there is some overlap between home health care and in-home services - some companies may offer both, some may offer only one
